CMS50 - Closing the Referral Loop: Receipt of Specialist Report

Measure Description: Percentage of patients with referrals, regardless of age, for which the referring provider receives a report from the provider to whom the patient was referred

Denominator: Number of patients, regardless of age, who were referred by one provider to another provider, and who had a visit during the measurement period

Numerator: Number of patients with a referral, for which the referring provider received a report from the provider to whom the patient was referred

Measure Calculation in ChARM EHR: AUTOMATIC

This measure is automatically calculated based on the referrals sent to other providers from ChARM EHR.

To satisfy this measure, use the Referral section in ChARM to create a Referral Out. Choose the receiving provider, enter the reason for referral and attach additional documents if required. You can use the Fax or Direct Message options to transmit the referral order to external providers.

Once you get the report from the receiving provider, import it under the ‘Response’ section of the corresponding Referral Order, to close the referral loop.
