Clinical Quality Measures
Measure Description: Percentage of patients aged 18 years and older with a BMI documented during the current encounter or during the previous twelve months AND with a BMI outside of normal parameters, a follow-up plan is documented during the encounter or during the previous twelve months of the current encounter
Normal Parameters: Age 18 years and older BMI => 18.5 and < 25 kg/m2
Denominator: All patients 18 and older on the date of the encounter with at least one eligible encounter during the measurement period
Numerator: Patients with a documented BMI during the encounter or during the previous twelve months, AND when the BMI is outside of normal parameters, a follow-up plan is documented during the encounter or during the previous twelve months of the current encounter
Measure Calculation in ChARM EHR: AUTOMATIC and MANUAL
This measure is calculated based on the BMI recorded in the Vitals section and interventions performed if the BMI is outside the normal range.
To satisfy this measure, for all patients with Age >= 18 years, record Weight and Height in the Vitals section. If the patient is overweight (BMI >= 25) or underweight (BMI<18.5) perform additional interventions as listed below and record them under 'Encounter > MU' section.

Denominator Exclusion: Patients will be automatically excluded if Patient has Diagnosis 'Pregnancy Dx' (Sample ICD10 codes : O00.00, O00.201, O10.213)
In addition, patients will be manually excluded based on the other conditions listed below in 'Encounter > MU' section.
