Create Feedback Form

You can measure the Patients' satisfaction and get their feedback on the treatment offered by creating a Patient Satisfaction Survey. As a first step, you have to create a 'Feedback Form' from the Settings > Questionnaire section.
Create Questionnaire Option

Click on the '+Questionnaire' button.
New Questionnaire

Enter the name of the form and choose the 'Questionnaire Type' as 'Feedback Form'.

Add the required questions using the components provided and save the Questionnaire.
