Long-Term Post-Acute Care (LTPAC)

Introduction

Long-Term Post-Acute Care (LTPAC) refers to the healthcare services for Patients who need extended recovery or ongoing support after a hospital stay. This care is typically delivered in specialized locations, such as Skilled Nursing Facilities, Rehabilitation Centers, etc.

CharmHealth EHR is currently integrated with LTPAC platforms such as PointClickCare (PCC) and MatrixCare, facilitating seamless data exchange between Long-Term Post-Acute Care (LTPAC) Facilities and Healthcare Providers.

How it Works

  1. A staff member at the LTPAC Facility adds a new Patient or updates the existing Patient details.
  2. The Patient information from the LTPAC platform is automatically pushed to CharmHealth EHR.
  3. The Patient record is created or updated under the corresponding Facility within CharmHealth and can be viewed in the Patient Dashboard.
  4. The Consulting Prescriber reviews the updated Patient details and documents the clinical Encounter in CharmHealth.
  5. Once the chart note is signed, the clinical documentation is automatically transferred to the respective LTPAC Platform.
  6. The note appears in the LTPAC platform as Progress Notes or a PDF document.
